With the start of perimenopause, my hormonal acne became a symptom that I have been trying to resolve for a while now. I am using blue light therapy and retinol, but the acne on my chin keeps coming back. After doing some research, the DIM (Diindolylmethane) supplement came up and got me intrigued. What is a DIM supplement?
Diindolylmethane (DIM) is a compound found in cruciferous vegetables like broccoli, cauliflower, cabbage, and kale. It changes how the body processes estrogens.
I love cruciferous vegetables and try to incorporate them in my meals as much as possible, but you’d need to eat roughly two pounds of raw cruciferous vegetables every single day to get a therapeutic dose of 100–300 mg. A typical diet only provides about 2-24 mg of DIM daily, which is why a supplement can be a good option for some perimenopausal women(15).
What Are the Benefits of DIM Supplements for Perimenopause?
DIM works by influencing how your body processes estrogen – and I don’t mean it “lowers estrogen,” which is an oversimplification you’ll see everywhere. Your body breaks down estrogen through several pathways, some protective, some problematic. DIM pushes your metabolism toward the protective ones. It helps your body handle the estrogen it already has.
This matters in perimenopause because progesterone drops first, while estrogen can swing up and down. That imbalance, called estrogen dominance, is what drives heavier periods, worse PMS, breast tenderness, bloating, hormonal acne, and stubborn midsection weight gain. DIM helps your body process that unopposed estrogen more efficiently rather than letting it build up through the wrong pathways.
This is backed by research. A 2024 study of over 19,000 women found that DIM users had higher levels of protective estrogen metabolites, lower levels of problematic ones, and a more favorable overall ratio (2). A 2025 follow-up in Menopause confirmed the same shift in over 1,400 women on HRT patches (1).
Based on the evidence, DIM may help with symptoms driven by estrogen dominance in perimenopause –
- heavy or irregular periods,
- hormonal acne (especially jawline breakouts),
- cyclical breast tenderness,
- PMS mood swings and irritability,
- bloating and water retention,
- midsection fat gain,
- It may also support breast health by shifting estrogen metabolism away from proliferative pathways – one trial in BRCA carriers found reduced breast tissue density after a year of supplementation (5).
One important thing to note: this is most relevant during early to mid perimenopause when estrogen is still actively circulating. If you’re in late perimenopause with low estrogen – think hot flashes, vaginal dryness, periods mostly gone – DIM may not be the right choice for you, and I’d recommend getting hormone testing before starting it.
What to look for in a DIM supplement
When selecting a Diindolylmethane (DIM) supplement, prioritize products with:
100–300 mg dosage, and third-party testing (such as Clean Label Project or independent lab verification.
DIM on its own is poorly absorbed. Look for supplements that use absorption-enhanced formulations – BioResponse DIM (BR-DIM) is the most studied and has roughly 50% higher bioavailability than plain crystalline DIM (12).
BioPerine (black pepper extract) and sunflower lecithin are absorption enhancers you’ll see in some formulations.
Look for “clean label” brands that are GMO-free, vegan, and free from fillers like magnesium stearate or titanium dioxide.

Who Should NOT Take DIM
Hard No
- Pregnant or breastfeeding women. Due to DIM’s hormonal effects, this is a clear contraindication. Stick to the DIM you naturally get from food.
- Women with hormone-sensitive cancers (breast, uterine, ovarian) – unless specifically recommended by your oncologist.
- Women with endometriosis or uterine fibroids – without doctor approval.
- People with severe liver or kidney disease.
Talk to Your Doctor First
- If you’re on HRT (hormone replacement therapy). A 2025 study found that DIM significantly altered estrogen profiles in women on estradiol patches (1). It could potentially reduce the effectiveness of your hormone therapy, which defeats the purpose of being on it. Your practitioner needs to know.
- If you’re on tamoxifen. Clinical trial data showed DIM decreased serum levels of tamoxifen metabolites, including endoxifen (the active form)(4). This could potentially reduce tamoxifen’s effectiveness.
- If you’re on birth control pills. DIM may interfere with how they work.
- If you take thyroid medication. DIM can impact thyroid hormone levels, which may require dosage adjustments.
- If you take diuretics (water pills). DIM can lower sodium levels, and combining it with sodium-depleting diuretics could cause dangerously low sodium.
- If you take medications metabolized by CYP3A4 or CYP1A2 enzymes. This includes some statins, anti-anxiety medications (like buspirone), and certain antihistamines (like fexofenadine). If you take any prescription medications, check with your pharmacist (11).
- If you’re in late perimenopause or menopause with genuinely low estrogen. If your primary symptoms are hot flashes, vaginal dryness, and bone density concerns from low estrogen, DIM could make things worse.
FAQ
Can DIM Help with Hormonal Acne?
Theoretically, yes, but there are no clinical studies that have specifically tested DIM for acne – I want to be upfront about that. But the mechanism behind it makes sense. DIM works on two pathways: it shifts estrogen metabolism in a way that may reduce sebum overproduction, and it appears to block DHT (the androgen most associated with hormonal acne) from binding to its receptors. It may also slow the growth of acne-causing bacteria on the skin (13).
The anecdotal evidence is huge – hormonal acne, especially those deep jawline and chin breakouts, is one of the most common reasons women try DIM, and many report real improvement. The research supports that DIM modulates the hormonal pathways involved in hormonal acne, but direct acne-specific trials haven’t been done yet.
When to take the DIM supplement?
Take it with food – preferably a meal containing some fat, as this improves absorption and reduces the most common side effect (stomach upset).
Expect it to take time. Most women notice changes after about 30 days, and it may take 2-3 months to see full effects on hormonal symptoms like acne and cycle regularity.
What are the side effects of DIM?
Most women tolerate DIM well, but here are some potential side effects:
Common (usually mild):
- Darkened urine (harmless – drink more water)(3)
- Increased bowel movements
- Mild stomach upset, gas, or nausea (usually resolves by taking it with food)
- Headaches (usually mild and temporary)
Less common:
- Changes in menstrual cycle timing or flow
- Temporary increase in hot flashes (especially if estrogen is already low)
- Skin rash (rare)
- Temporary worsening of acne during the first couple of weeks (sometimes called a “purge” as estrogen metabolism shifts)
Reported at high doses (300+ mg):
- Nausea and vomiting
- More pronounced headaches
- One case report of visual impairment after excessive daily intake for 2 months (resolved after stopping)(14).
If you experience anything beyond mild, temporary side effects – stop taking it and talk to your doctor.

I want to be honest about the studied effects of DIM for perimenopausal women; there are quite a few gaps. Despite hormonal acne being one of the most popular reasons women take DIM, and the reason I want to test it out, direct evidence for this use doesn’t exist yet (13).
